Reliable transportation can make it easier for your loved one to participate in meaningful activities, receive daytime support, and stay connected with others. Home Away From Home helps members and families understand and coordinate transportation options through DELGO Community Transit.

Introduction

Making the Trip More Manageable

Getting to an adult day center should not be the most difficult part of receiving support. Our team can help you understand the transportation process, provide the information needed for trips to and from our center, and answer questions about coordinating a member’s attendance schedule. Transportation vehicles and drivers are provided by DELGO Community Transit. DELGO makes all final decisions regarding rider registration, eligibility, trip availability, routes, pickup times, fares, passenger assistance, and ride approval.

How It Works

How Transportation Coordination Works

1. Tell Us You Need Transportation Help

Contact Home Away From Home and let us know that transportation may be needed. We will ask a few basic questions about the member’s attendance schedule and the type of assistance being requested.

2. Complete DELGO’s Rider Requirements

Members must meet DELGO’s current registration and program requirements before transportation can be scheduled.

Our team can direct you to the appropriate DELGO information and help you understand what to do next.

3. Request the Ride

The member or an authorized representative may request transportation directly through DELGO.

Depending on the member’s needs and Home Away From Home’s approved procedures, our team may also assist with communicating attendance details, center hours, and ride changes to DELGO after appropriate authorization has been obtained.

4. Confirm the Attendance Schedule

Keep both Home Away From Home and DELGO informed of schedule changes, absences, vacations, late arrivals, or early departures.

Notifying Home Away From Home does not automatically cancel or change a DELGO ride.

Home Away From Home can help members and families:

  • Locate current DELGO registration and rider information.
  • Understand which transportation questions should be directed to DELGO.
  • Confirm our center’s address, entrance, operating hours, and planned attendance schedule.
  • Communicate a member’s approved attendance days to DELGO when authorized and operationally appropriate.
  • Identify questions to ask about wheelchairs, walkers, escorts, companions, or passenger assistance.
  • Coordinate communication when an enrolled member’s program schedule changes.
  • Determine whom to contact when a member will be absent or delayed.

Our role is to support communication and coordination. DELGO remains responsible for operating and approving the transportation service.

What DELGO Manages

DELGO Community Transit is responsible for:

  • Rider registration and eligibility.
  • Acceptance or denial of trip requests.
  • Ride availability and scheduling.
  • Pickup and return windows.
  • Drivers, vehicles, routes, and dispatch.
  • Service areas and destination limitations.
  • Fares, payment methods, and transportation charges.
  • Wheelchair-accessible vehicle assignments.
  • Escort or companion arrangements.
  • Curb-to-curb or requested passenger assistance.
  • Weather-related suspensions, delays, and cancellations.

Because transportation is a shared-ride service, members may travel with other passengers. Travel may take longer than it would in a private vehicle.

An enrolled member or authorized representative may ask Home Away From Home for help communicating with DELGO about transportation to and from the center.

  • Confirm the identity of the person making the request.
  • Confirm that the person is the member or is authorized to act for the member.
  • Obtain permission to share limited transportation-related information.
  • Confirm the member’s attendance schedule and transportation needs.
  • Explain which requests must still be made directly to DELGO.

Transportation coordination does not transfer responsibility for the ride to Home Away From Home. DELGO retains final authority over all transportation decisions.

With the member’s or authorized representative’s permission, our staff may assist with requesting, confirming, changing, or canceling DELGO transportation for trips to and from Home Away From Home.

Accessibility and Mobility

Mobility Devices and Passenger Assistance

Transportation needs are different for every member.

Tell DELGO about any wheelchair, walker, mobility device, escort, entrance, or passenger-assistance need when requesting a ride.

DELGO offers wheelchair-accessible vehicles, but an accessible vehicle must be requested and arranged through DELGO. Door-to-door or other passenger assistance may also require advance notice and DELGO approval.

Transportation accessibility and center eligibility are separate considerations. Home Away From Home will independently review whether our center can safely support a prospective member’s care, mobility, transfer, supervision, and personal-assistance needs.

Information Families Should Prepare

Before requesting transportation, be prepared to provide or confirm:

  • The member’s name.
  • The pickup location and correct entrance.
  • A reliable contact number.
  • Home Away From Home’s address and designated entrance.
  • The planned arrival and departure times.
  • The days the member expects to attend.
  • Any wheelchair, walker, or mobility-device information.
  • Whether an escort or companion is needed.
  • Any passenger-assistance request.
  • The responsible contact for schedule changes.
  • DELGO rider registration information, when applicable.

Do not send Social Security numbers, payment information, medical records, insurance numbers, rider identification numbers, or copies of identification through the general website contact form.

Scheduling and Availability

Plan Transportation Early

Transportation is subject to registration, scheduling requirements, vehicle capacity, shared-ride availability, and DELGO approval.

We recommend beginning the process as early as possible.

Registering for Home Away From Home, scheduling a tour, or submitting a transportation request to our team does not automatically schedule a ride.

For current scheduling deadlines, fares, service areas, and rider requirements, review DELGO’s official rider information or contact DELGO directly.

With the member’s or authorized representative’s permission, our staff may assist with requesting, confirming, changing, or canceling DELGO transportation for trips to and from Home Away From Home.

Arrival, Departure, and Schedule Changes

Keeping Everyone Informed

Please notify both organizations when plans change:

  • Contact DELGO about the status, cancellation, or modification of a transportation reservation.
  • Contact Home Away From Home when the member will be absent, late, arriving early, or leaving at a different time.
  • Do not assume that contacting one organization automatically updates the other.
  • For immediate vehicle-location or pickup questions, contact DELGO.
  • For questions about attendance or the member’s day at the center, contact Home Away From Home.

During scheduled program hours, members arriving through coordinated transportation should use our designated entrance. A member of our team will assist with the center check-in process.

  • Does Home Away From Home provide the vehicle or driver?

    No. Transportation is operated by DELGO Community Transit.

  • Can Home Away From Home arrange transportation on a member’s behalf?

    We may be able to assist an enrolled member or authorized representative with transportation coordination, depending on the assistance requested and our approved procedures.


    Before communicating with DELGO on a member’s behalf, we may need to confirm the person’s authority and obtain appropriate permission.


    DELGO remains responsible for accepting, scheduling, changing, or denying the ride.

  • Does submitting the transportation form schedule a ride?

    No. The form tells Home Away From Home that you would like assistance. It does not register a rider with DELGO, create a transportation reservation, confirm a pickup time, or guarantee a ride.

  • Who determines whether a member qualifies for transportation?

    DELGO determines eligibility for its transportation programs. Eligibility for transportation is separate from eligibility to attend Home Away From Home.

  • Is transportation guaranteed?

    No. Transportation is subject to DELGO registration, scheduling requirements, service area, capacity, shared-ride availability, and trip approval.

  • Is this a private ride?

    DELGO operates shared-ride transportation. A member may travel with other passengers going in the same general direction, and the trip may take longer than travel in a private vehicle.

  • Are wheelchair-accessible vehicles available?

    DELGO operates wheelchair-accessible vehicles. Members must tell DELGO about wheelchairs, walkers, mobility devices, and passenger-assistance needs when requesting transportation. Home Away From Home must separately determine whether the center can safely support the member’s mobility, transfer, supervision, and personal-care needs.

  • Can a family member or caregiver ride with the member?

    DELGO may allow an escort or companion when a seat is requested in advance.


    DELGO determines availability, requirements, and any applicable fare.

  • Is door-to-door assistance available?

    DELGO’s standard service is generally curb-to-curb.Additional passenger assistance may be available when requested in advance and approved by DELGO.

  • Who should I contact if the vehicle is late?

    Contact DELGO for vehicle status, dispatch, pickup, and transportation-delay questions. Please also notify Home Away From Home if the delay will affect the member’s attendance.

  • Who should I contact to cancel a ride?

    Follow DELGO’s current cancellation procedure to cancel the transportation reservation. Contact Home Away From Home separately to report the member’s absence.

  • Are transportation fares included in Home Away From Home’s fees?

    Transportation charges are set and collected by DELGO and are separate from Home Away From Home’s program fees unless a different arrangement has been confirmed in writing.

  • Can transportation be used for a tour or open house?

    Transportation may be available to eligible, registered riders, subject to DELGO scheduling and availability. Contact us before selecting a visit date so we can provide the correct center information. Scheduling a tour or open house does not reserve transportation.
